OPEN ENROLLMENT REQUIREMENTS

 

When is open enrollment?

Open enrollment will now take place from November 1 – 30, 2016.

Who does this change effect?

All Worcester County Public Schools employees, even those not participating in any type of insurance coverage.

What do I need to do?

Anyone who is not currently enrolled with a health, dental and/or vision insurance plan must complete an insurance waiver form.

What paperwork do I need to provide?

Employees not participating in coverage will need to provide the completed insurance waiver form. In order to be compliant with the Affordable Care Act (ACA), this form must be completed every year. 

Employees wishing to enroll or add dependents to the insurance benefits must provide a copy of his/her Social Security card, the completed application for each insurance with which you plan to participate and the appropriate paperwork. Examples of required paperwork are below:

Husband / Wife Coverage
Parent / Child Coverage
Family Coverage
  • Affidavit of Disclosure
  • Copy of Marriage Certificate
  • Copy of Spouse's Social Security Card
  • Affidavit of Disclosure
  • Copy of Birth Certificate, Adoption Papers, etc.
  • Copy of Child(ren)'s Social Security Card(s)
  • Affidavit of Disclosure
  • Copy of Marriage Certificate
  • Copy of Birth Certificate, Adoption Papers, etc.
  • Copy of Spouse's and Child(ren)'s Social Security Card(s)

When do I turn in this paperwork?

Please submit all paperwork by the end of the day on Wednesday, November 30, 2016, to the Human Resources Department.
